Dr. Cloyd D. Van Hook, 93, died at home Thursday, Jan. 26, 2012. A graveside service will be held at 11 a.m. Monday, Jan. 30, at Lindquist’s Washington Heights Memorial Park, 4500 Washington Blvd., Ogden, where military honors will be accorded. Services entrusted to Lindquist’s Ogden Mortuary, 3408 Washington Blvd. In lieu of flowers, contributions can be made to Rocky Mountain Hospice, Utah Cancer Society or the First Baptist Church in Ogden.
