ELKO, Nev. -- Nevada workplace safety officials have fined a Utah company $5,000 for violations found after a worker fell to his death through a skylight opening during a commercial building renovation job in Elko.
A Nevada Occupational Safety and Health Administration report faults TRC Construction of Midway, Utah, for lapses in building guidelines following the death of Carlos Islas of Elko.
Islas died when he fell from the roof more than 24 feet to the concrete floor of a Mountain City Highway building being refurbished to house a Jo-Ann Fabrics and a Ross Dress for Less.
The Elko Daily Free Press reports that OSHA found Islas wasn't wearing restraint gear while working around the unprotected 3-foot by 4-foot opening.
TRC Construction didn't respond to a call seeking comment.
